4 Year old Healthy Kids Checks
The aim of a healthy kids check is to ensure every child has a basic check to see if they are healthy, fit & ready to learn when they start school.
The practice nurse gives every 4 year old a Health Kids Pack at the time of their appointments. The pack gives the children & parents ideas on Healthy eating (lunch box ideas), dental health and physical activity.
Parents are asked if they have any concerns at the time of the consultation and where appropriate referrals can be put in place for any problems identified.
45-49 year old check
This check aims to assist in the early detection and prevention of chronic diseases. It enables prevention strategies.
The check involves taking your history, noting your current health issues and risk factors as well as looking at your family history.
The health check will look at:
• Lifestyle risk factors such as smoking, nutrition, alcohol consumption, nutrition and physical activity
• Family history of any chronic disease
Overall this check aims to assess your lifestyle, detect issues early, & prevent disease, thus improving your long term health outcomes. Getting in early can have a big impact later on!
Over 75 Health Checks
A health assessment of an older person is an in-depth assessment of a patient aged 75 years and over. It is an annual assessment of physical, psychological and social well-being and offers preventative health care and education. The practice nurse conducts the assessment and following this you will have a consultation with your GP to discuss the results.
