General Practitioners & Practice Nurses Offer the following Age Specific Health Checks
45-49 year old check
This check aims to assist in the early detection and prevention of chronic diseases. It enables prevention strategies.
The check involves taking your history, noting your current health issues and risk factors as well as looking at your family history.
The health check will look at:
• Lifestyle risk factors such as smoking, nutrition, alcohol consumption, nutrition and physical activity
• Family history of any chronic disease
Overall this check aims to assess your lifestyle, detect issues early, & prevent disease, thus improving your long term health outcomes. Getting in early can have a big impact later on!
Heart health check
This health check is available to patients aged from 45 years to 49 years inclusive.
It involves the Doctor assessing your risk of developing heart disease, which is still the most common cause of death in Australia.
The Doctor will ask questions, conduct an examination including blood pressure measurement and order some blood tests e.g. cholesterol. The Doctor will provide preventative health care advice and information.
Over 75 Health Checks
A health assessment of an older person is an in-depth assessment of a patient aged 75 years and over. It is an annual assessment of physical, psychological and social well-being and offers preventative health care and education. The practice nurse conducts the assessment and following this you will have a consultation with your GP to discuss the results.
